Hauora Assessment & Goal Planning Tool[edit | edit source]
Overview[edit | edit source]
The Hauora Assessment and Goal Planning Tool is a structured, culturally responsive process used within Kotahi to assess client wellbeing and co-create meaningful goals. It draws on models such as Te Whare Tapa Whā and Pacific wellbeing frameworks, supporting holistic engagement across multiple domains of life.
This tool supports:
- Strengths-based assessments
- Culturally meaningful outcome tracking
- Client-led goal setting
- Progress monitoring across services
Purpose[edit | edit source]
- To assess a client or whānau member’s overall wellbeing across multiple domains
- To identify aspirations and co-develop goals with clear, measurable outcomes
- To inform service delivery and enhance reporting accuracy
- To track client progress from entry to exit through visual and narrative tools
Hauora Domains[edit | edit source]
Domains represent key areas of wellbeing. Each domain contains:
- Outcome Areas – specific themes within a domain
- Outcomes – measurable results used for tracking change
Example Domain Structure[edit | edit source]
- Domain
- Financial Security
- Outcome Area: Basic Needs
- Outcome: Able to afford food and housing
- Outcome Area: Future Planning
- Outcome: Has emergency savings or plan
Domains may include:
- Whānau / Family Relationships
- Cultural Identity and Belonging
- Physical Health
- Mental and Emotional Wellbeing
- Spiritual Wellbeing
- Safety and Protection
- Education and Employment
- Financial Wellbeing
- Housing and Environment
- Social Connection
- Self-Determination and Autonomy
Setting Up Domains (Admin)[edit | edit source]
Admins can define and customise domains via:
- Go to Configuration Menu → Hauora Domains
- Create a Domain → Add Outcome Areas → Add Outcomes
- Ensure each item is marked Active and logically grouped
Each Domain can contain multiple Outcome Areas and Outcomes, which can be linked to client goals.
Building the Assessment[edit | edit source]
To create an assessment:
- Go to Hauora Assessment Types
- Create a new type and assign it to relevant services
- Link it to existing Domains
- Add custom questions per Domain (with help text if needed)
- Publish the assessment for staff to use
Completing the Assessment (Staff)[edit | edit source]
- Staff access the Hauora Assessments tab on a client’s profile
- Select Create New Hauora Assessment
- For each question:
- Select alignment level (Strongly Disagree to Strongly Agree)
- Add optional notes
- Use the domain summary area to reflect on overall insights
Scoring is visualised using percentages and a spider graph, showing baseline and follow-up assessments side by side.
Creating the Goal Plan[edit | edit source]
After completing an assessment:
- Navigate to the Hauora Goal Plans section
- Click Create Plan
- Enter client aspirations (free text)
- Add one or more Goals:
- Description
- Start and Due Dates
- Service
- Domain → Outcome Area → Outcome
- For each Goal, add Actions:
- Assign staff or whānau responsibility
- Set timeframes and tasks
- Link to relevant whānau if needed
Reviewing and Updating Plans[edit | edit source]
- To review a goal or action: double-click the item
- Update status, notes, and review date
- Save changes to track progress
- Visual indicators show goal and action completion
Reports and Oversight[edit | edit source]
- Staff can view client summary pages with assessment and goal plan data
- Managers have admin-level reporting access to view:
- Overdue goals
- Staff activity
- Client engagement across domains
