= Hauora Assessment & Goal Planning Tool = == Overview == The Hauora Assessment and Goal Planning Tool is a structured, culturally responsive process used within Kotahi to assess client wellbeing and co-create meaningful goals. It draws on models such as Te Whare Tapa Whā and Pacific wellbeing frameworks, supporting holistic engagement across multiple domains of life. This tool supports: * Strengths-based assessments * Culturally meaningful outcome tracking * Client-led goal setting * Progress monitoring across services == Purpose == * To assess a client or whānau member’s overall wellbeing across multiple domains * To identify aspirations and co-develop goals with clear, measurable outcomes * To inform service delivery and enhance reporting accuracy * To track client progress from entry to exit through visual and narrative tools == Hauora Domains == Domains represent key areas of wellbeing.
